Names
Married Surnames
To add a married surname to a valid passport that was issued in you surname at birth, submit:
• a signed letter requesting the addition of your married surname to your passport,
• your marriage certificate (a photocopy is accepted),
• your valid passport, and
• the fee of CAN$12
To obtain a passport in your married surname you must apply for a new passport.
Spousal Common-law Surnames
To add a spousal common-law surname to a valid passport that was issued in your surname at birth, submit:
• a signed letter stating that you are:
• in a conjugal relationship,
• residing at the same address and the co-habitation has been for longer than one year, and
• requesting the addition of your spousal common-law surname to your passport
• your valid passport
• the fee of CAN$12
To obtain a passport in your spousal common-law surname you must apply for a new passport.
Legal Name Change
To obtain a passport in your new name, submit,
• an application form completed in full,
• all documentation as requested on the application form,
• one of the following:
• the legal change of name document,
• the adoption order, or
• the birth certificate or citizenship certificate issued in the new name
• For a change of name under any other circumstances you must apply for a new passport.
Place of birth
Your place of birth must be provided on the application form. If you do not wish the place of birth to appear in the passport, a written request must accompany the application. You should contact the consulate or embassy of every country you plan to visit to ensure that you will be admitted in the passport does not include your place of birth.
Signature
Your signature must not exceed the inner limits of the signature box and must appear on all three pages.
If you were born in Canada, you must provide an original ofeither:
• a Canadian birth certificate (issued in Canada by the province or territory of your birth), or
• a Certificate of Canadian Citizenship.

If you were born outside of Canada, you must provide one of the four Canadian citizenship documents below:
• Certificate of Canadian citizenship
• Certificate of Naturalization in Canada
• Certificate of Retention of Canadian citizenship
• Certificate of Registration of Birth Abroad
To avoid any delays, indicate in section 4 of the application form if you acquired citizenship of another country before February 15, 1977.
Birth certificates
Birth certificates are available from the Registrar of Vital Statistics in the province or territory of your birth. For more information on how to obtain a birth certificate, contact yourCanadian Vital Statistics Office.
Since November 26, 2001, only the following identity documents are accepted as proof of Canadian citizenship forCanadians born in the Province of Quebec.
• a birth certificate issued after January 1, 1994, by le Directeur de l'état civilin the Province of Quebec
• a Certificate of Canadian Citizenship
Birth certificate for children under 16 years of age
For children whose parentswere not married at the time of the child's birth, Passport Canada requires a long-form birth certificate showingboth parents' information.
For children whose parentswere married at the time of the child's birth, Passport Canada recommends a long-form birth certificate showingboth parents' information.

Dual Citizenship
Canadian law permits Canadians to have more than one nationality. However, your Canadian citizenship may not be recognized in the country of your other citizenship, and the authorities of that country may prevent Canada from providing you with consular assistance. This is especially true if you did not use your Canadian passport when entering that country.
Documents to support identity
At leastONE document to support your identity and the name to appear in the passport must be submitted with your application form. Further documentation may be required.For a child, only the proof of Canadian citizenship is required.
Documentation to support your identity must be issued by a federal, provincial or municipal authority, must be valid and must include the bearer's name and signature. For example:
• Provincial driver's licence
• Provincial health care card
• Other provincial identification card
• Certificate of Indian Status
• Firearms Licence - Possession - Acquisition
• Old Age Security card
• Other federal identification card
• Federal, provincial or municipal employee identification card
• Canadian passport*
* New: a previous Canadian passport may be used as supporting identification provided that all of the following conditions are met:
• the passport is still valid or submitted up to one year after its expiry date;
• the passport is submitted with the application form. Photocopies will not be accepted; and
• the name indicated in the passport is the same as the one on the application form.
You may submit original documents. They will be returned to you.If you are submitting photocopies (both sides), your guarantor must sign and date the photocopies to indicate the originals have been seen.
If you are submitting photocopies (both sides) but are using form PPTC 132, "Statutory Declaration in Lieu of Guarantor", the official who signs the "Declaration" must also sign and date the photocopies of your identification to indicate the originals have been seen.
